Does MM Card go onto your medical records?

Im 21 years old and i broke my back and neck 2 years ago and was thinking about getting my Medical Marijuana card, but i'm not sure if marijuana would help with my back pain so i don't want to commit to it for a year and not be able to get my pain medication for a year. So I was wondering does anybody know if I got my recomendation would my normal doctor for my back know about my marijuana card and does having the MM card go on your medical records?
